個人情報の取扱いに関する同意書
－ Consent to Use of Personal Information －
日本収益不動産株式会社（以下 ｢当社｣ という）に個人情報をご提供いただくにあたり、以下の内容につきましてご同意の上、ご提供いただきますようお願いいたします。
When providing personal information to Japan Investment Property Co., Ltd. (hereinafter referred to as “our company”), please agree to the following terms and conditions before providing your information.

取得目的と利用 当社は、宿泊申込者及び緊急連絡先予定者の個人情報について、宿泊予約およびチェックインに関する業務、外国からのお客様（代表者）のパスポートはご本人確認のため、必要なご連絡（チェックイン方法をお伝えする）のため、また、アフターサービス等の実施のために適正に利用することを目的としてます。
Purpose of collection and use: Our company will use the personal information of accommodation applicants and emergency contacts appropriately for the purposes of accommodation reservations and check-in procedures, the passport of foreign guests (representatives) to verify their identity, to contact them as necessary (to inform them of check-in procedures), and to provide after-sales service, etc.

情報提供の任意性 必ず宿帳にハウスルールを確認の上、お名前、年齢、性別、職業、国、住所、電話番号、メールアドレス等の情報をご記入いただきます。未成年（18歳未満）の方のみの宿泊には、未成年者の宿泊に対する保護者同意書が別途必要となります。予約の際に必要となる情報がご提供いただけない場合は、予約手続きができません。
Voluntary provision of information: Please be sure to check the house rules in the guest book before filling in information such as your name, age, gender, occupation, country, address, telephone number, and email address. If only minors (under 18 years of age) are staying, a separate parental consent form for minors to stay is required. If you are unable to provide the information required for making a reservation, you will not be able to proceed with the reservation.

個人情報の取扱いの委託について 取得した個人情報の取扱いの全部又は、個人情報の利用目的の達成に必要な範囲で、当社のグループ会社、取引先などに全部または一部を委託することがあります。その場合は、委託先に対して、適切な安全対策を講じている事等の選定・評価を実施し、委託契約等を締結し安全管理に努めます。
Regarding outsourcing of handling of personal information: We may outsource all or part of the handling of acquired personal information to our group companies, business partners, etc., to the extent necessary to achieve the purpose of using the personal information. In such cases, we will select and evaluate the outsourcee to ensure that they have taken appropriate safety measures, and will enter into an outsourcing contract, etc., and strive to manage safety.
4.
情報の開示・訂正・削除 旅館業法に基づき、作成日から3年以上の保存が義務付けられています。外国人の宿泊者については、氏名、住所、職業に加え、国籍と旅券番号の記載と旅券（パスポート）の写しの保存します。個人情報の開示、訂正または削除の依頼を受けた場合は、当社の手続き規程に従って速やかに対応します。
Under the Inns and Hotels Act, we are required to keep personal information for at least three years from the date of creation. For foreign guests, we will keep their name, address, occupation, nationality, passport number, and a copy of their passport. If we receive a request to disclose, correct, or delete personal information, we will respond promptly in accordance with our company’s procedural regulations.
5.
個人情報の第三者提供について 取得した個人情報は法令等による場合を除いて第三者に提供することはありません。
Regarding provision of personal information to third parties: Personal information acquired will not be provided to third parties except when required by law.
